(Brooklyn, NY)–HeartShare Wellness, a comprehensive medical and therapeutic clinic for children and adults with intellectual and developmental disabilities, has experienced several substantial changes.
First, HeartShare Wellness’s Article 28 medical clinic is now part of Metro Community Health Centers’ network of five health clinics in Brooklyn, Queens, Staten Island, and the Bronx. At the time of this transfer, the clinic also has opened a greatly expanded medical suite, which includes three state-of-the art dental rooms.
The transfer of the clinic was necessary due to detrimental rate changes that occurred 3 years ago. In becoming a Federally Qualified Health Center (FQHC), however, the clinic is now a full service health center resource for those outside the developmental disabilities community, including families from the community, particularly those on Medicaid.
Secondly, HeartShare Wellness is still running its Article 16 Clinic, which provides therapies and counseling to people with intellectual and developmental disabilities. The Article 16 clinic, located at 177 Livingston and at several satellite locations throughout Brooklyn and Queens, can now accommodate up to 19,000 patients each year.
Lastly, HeartShare Wellness’s Health Home Targeted Case Management Program remains strong and continues to grow. The Health Home Program facilitates care coordination for children and adults with two or more chronic conditions, such as HIV/AIDS or diabetes.
