Specializing care for underserved populations

HeartShare Wellness, Ltd., established in 2000, offers evaluations and therapy for children and adults with intellectual and developmental disabilities, as well as a targeted case management program for those with chronic conditions. Patients are met with specialized knowledge, sensitivity and compassion.

Article 16 Clinic

The HeartShare Wellness Article 16 Clinic is a therapeutic clinic for children and adults with intellectual and developmental disabilities. The clinic offers speech, occupational and physical therapies, individual and group counseling, psychological and psychosocial evaluations, as well as sexuality assessments. Those services are now available in day habilitation programs at HeartShare, Guild for Exceptional Children, and Heaven’s Home Community Services.

Health Home

Health Home is a Targeted Case Management Program offering care coordination and resources for children and adults with two or more chronic conditions throughout New York City.

The goal of the Health Home program is to make sure its members get the care and services needed. This may mean fewer trips to the emergency room or, less time spent in the hospital. It could mean getting regular care and services from doctors and providers. Or, finding a safe place to live, and a way to get to medical appointments.

Once you are enrolled in a Health Home, you will have your own care manager. The care manager will work with you to set up a care plan and appointments and get the services you need to put you on the road to better health. Some of the services may be:
